Plenty of surf schools in Tamarindo offer surf lessons. The best are Tamarindo Surf School and Iguana Surf.

I found Iguana Surf has a ratio of 7/1 student p/ instructors which i think is too crowded and both were expensive
Costa Rica Surf Club (http://www.costaricasurfclub.com)has a ratio of 5/1 and charge less for the same service, also there are plenty of surfboards for rent or buy.
